The Community Health Worker (CHW) plays a vital role in improving community health by facilitating access to healthcare resources, providing health education, and advocating for patients. As a liaison between healthcare providers and the community, the CHW empowers individuals by connecting them to services, offering support, and helping them overcome barriers to achieving better health. This position requires a compassionate, resourceful, and self-motivated individual dedicated to making a meaningful impact on the health and wellbeing of individuals and families facing unique challenges in the community.

Responsibilities Include but not limited to:

  • Community Outreach and Engagement: Build and maintain relationships with community members, especially those at high risk, to identify and address health needs. Engage and recruit individuals who may benefit from healthcare services and social support.
  • Care Coordination and Advocacy: Serve as an intermediary between healthcare providers, social services, and the community to facilitate access to services. Advocate for clients' health and social needs, ensuring their voices are represented in healthcare settings.
  • Health Education: Deliver culturally appropriate health education on topics such as chronic disease management, preventive care, and healthy living to empower individuals and communities to take control of their health.
  • Support Services and Follow-Up: Assist clients in overcoming barriers to care, including transportation, housing, and food insecurity. Provide emotional support, guidance, and ongoing follow-up through phone calls, home visits, or visits to other settings to ensure patient needs are met.
  • Enrollment Assistance: Assist individuals with enrolling in Qualified Health Plans (QHPs) by reviewing health coverage options, completing eligibility and enrollment forms, and ensuring information is provided in a culturally and linguistically appropriate manner.
  • Documentation and Reporting: Maintain accurate records of client interactions, services provided, and outcomes. Ensure compliance with state and organizational guidelines, and submit regular reports as required.
  • Collaboration and Program Development: Work with healthcare providers, social workers, and community organizations to develop and implement programs addressing community health needs. Participate in health fairs, workshops, and other outreach events to raise awareness and promote health initiatives.
  • Problem-Solving and Barrier Removal: Collaborate with patients and care teams to identify and resolve challenges to achieving healthcare goals, including navigating complex healthcare systems and managing chronic conditions.
  • Perform other duties as assigned to support the mission and goals of the organization.

Education and/or Experience

  • Education: High school diploma or equivalent required. Bachelor’s degree in a related field preferred. Certification as a Community Health Worker (CHW) is preferred or willingness to obtain certification within a specified timeframe.
  • Experience: Experience in community health, human services outreach, or healthcare is preferred. Experience with customer service, patient coaching, community relations, and public speaking is a plus.
